This is the second online workshop in a series of 6 bundle workshops focusing on the Living and Ageing Well and Dying Well service across BaNES, Swindon and Wiltshire.

The BSW Integrated Care System is currently reviewing a large range of Community Based Health care services that are all coming up for re-tender during 2024.
BSW is committed to delivering integrated care services in line with the Care Model that has been developed which is driven by the need to take a ‘strengths based’ approach and to deliver care services that are personalised around the specific needs of the individual and their families.
Currently there are inconsistencies as to how community services are delivered across BSW with some services being delivered in a manner which are focussed on the service provider, rather than placing the individual at the centre of care.
BSW is taking the opportunity to review all of these services leading up to the re-tendering dates with a view to refreshing the thinking on what is needed going forward taking into account:
1. What are the current and future needs of the population?
2. What outcomes are we striving to achieve?
3. What services will best enable us to deliver those outcomes?
4. Which organisations should be responsible for which services?
There are a series of workshops coming up very shortly which we are keen to make sure the VCSE voice, knowledge and capability is represented at. The workshops fall under different themes so some may be more relevant to you and your work than others – or you may feel not relevant at all.
The following four face to face workshops, likely to be on 10th, 14th, 21st and 22nd February, venue tbc:
Bundle 5 – Supporting Self Care & Prevention
Bundle 6 – Proactive Care & Support
Bundle 7 – Responding to Urgent Needs
Bundle 8 – Rehabilitation & Recovery from Ill Health and/or crisis
