The Integrated Care System is the new health and care system which oversees the broader health and wellbeing strategy for Bristol, North Somerset and South Gloucestershire. 


There are 6 place-based partnerships working at a local level to deliver care in the community.  These partnerships bring a range of organisations together to best meet the needs of the individual. 


As an Integrated and Personalised Team Coordinator you will work across two Primary Care Networks  harmonising delivery of mental health support for individuals.
